Who we are
Nova Chiropractic & Wellness is operated by Dr. Dejan Jelic Chiropractic Professional Corporation, 1614 Lesperance Road, Building A2, Tecumseh, Ontario. Under Ontario's Personal Health Information Protection Act, 2004 (PHIPA), Dr. Dejan Jelic is the health information custodian responsible for the personal health information collected by this clinic.
What we collect
- Identity and contact details - name, date of birth, address, phone, email
- Health history, examination findings, X-ray images and their analyses, care plans, and visit records
- Insurance and billing information, including extended-health plan details used to bill your insurer and, where applicable, motor-vehicle-accident claim information
- Communication preferences, such as consent to appointment reminders by email or text
We collect this information directly from you, or from other health providers with your consent.
Why we collect it
- To assess your condition and provide safe, appropriate chiropractic care
- To schedule visits and send confirmations and reminders you have asked for
- To communicate with your insurer when you ask us to, and to process payments
- To communicate with other health professionals involved in your care, with your consent
- To meet our legal and professional obligations, including record-keeping standards of the College of Chiropractors of Ontario
We do not sell your information, and we do not use your health information for marketing.
Consent
By providing your information and receiving care, you consent to its collection and use for the purposes above. You may withdraw or limit your consent at any time, subject to legal and professional record-keeping requirements - for example, you can opt out of reminder messages while remaining a patient. Some limits on consent may affect our ability to provide care or communicate with your insurer on your behalf; we will explain any consequences before proceeding.
How we protect it
Your records are stored in secure, access-controlled clinical systems. Access is limited to clinic staff who need it to provide your care or administer your account, and every member of our team is bound by confidentiality obligations. Paper records, where they exist, are kept locked on the premises.
How long we keep it
Chiropractic records in Ontario must be retained for a minimum period set by the College of Chiropractors of Ontario - generally at least ten years from your last visit, or ten years from a minor patient's 18th birthday. After the retention period, records are securely destroyed.
Your rights
- You may request access to your health record and receive a copy within a reasonable time
- You may request correction of information you believe is inaccurate or incomplete
- You may ask who has accessed your information, and request limits on how it is shared
- You may withdraw consent to non-essential uses, such as reminders, at any time
This website
Browsing this website does not require an account and does not collect your health information. Online booking is handled by a third-party scheduling provider, which processes the details you enter under its own privacy terms. The map on our home page is provided by Google. When it loads, Google may receive your IP address and other technical details from your browser, and handles them under its own privacy policy.
